As a Provider I will...

Provide a licensed clean, safe day care home.

Support and encourage the physical, social, intellectual, creative and emotional development of your child

Provide your child with nutritional meals and snacks

Treat your child with respect and affection

Provide you with a year-end total for your income tax purposes

Notify you of any suplies needed in a timely manner

Have an open-door policy to provide you with the comfort to "drop-in" or visit as you desire (Please take naptime and mealtime into  consideration)

Be open to communicaton regarding your child, my care and my home

As the Parent/Guardian I will...

Supply the day care with any special foods, clothing, and equipment as necessary for the care of my child

Notify the day care immediately of any changes in phone numbers, addresses, and emergency contact information

Notify the day care of any days my child will not be in attendance due to illness or vacation by calling in advance and leaving a message if necessary

Notify the day care of any changes in schedule that could result in a change of drop-off or pick-up time for my child

Inform the day care of any changes in my child's life that may cause a change in behavior, i.e. divorce, death in the family, etc.

Keep my child's supply needs met (extra clothes, diapers, wipes, ointments, etc.)

Voice all concerns regarding the day care home, activities, food, songs, games, etc.

Offer to provide help, ideas, and supplies as available

Keep communication open and be willing to discuss all issues pertaining to the care of my child

Ring the doorbell and enter with my child as the provider is sometimes in the middle of an activity with other children and cannot leave them unattended

Abide by prearranged hours and if I am running late I will contact the day care as soon as possible

Keep financial agreements according to the contract and should there be any questions regarding my bill, I will discuss it with the day care provider

Pay my bill on time as I understand the day care has bills also

Bring a change of clothing for my child

Provide the following for my child, if necessary, diapers, wipes, sunblock, formula, baby food, change of clothing, bottles, and paperwork
