As the Provider, I will…
Provide a licensed, clean, safe day care home.
Support and encourage the physical, social, intellectual, and emotional development of your child.
Provide your child with nutritional meals and snacks.
Treat your child with respect and affection.
Provide you with a year-end total for your income tax purposes.
Notify you of any supplies needed in a timely manner.
Have an open door policy to provide you with the comfort to ‘drop-in’ or visit as you desire. (Please
take nap time into consideration)
Be open to communication regarding your child, my care and my home.
As the Parent/Guardian, I will
Supply me with any special foods, clothing, and equipment as necessary for the care of your child.
Notify me immediately of any changes in phone numbers, addresses, and emergency contact information.
Notify me of any days not in attendance due to illness or vacation by calling in advance.
Notify me of any changes in schedule that will result in a change of drop off or pick up time for your
child.
Inform me of any changes in your child’s life that may cause a change in behavior, i.e.: divorce, death
in the family, etc.
Keep your child’s supply need met. (Extra clothes, diapers, wipes, ointments, etc.)
Voice all concerns regarding my home, activities, food, songs, games, etc. Offer to provide help, ideas,
and supplies as available.
Keep communication open and be willing to discuss all issues pertaining to the care of your child.
Ring the doorbell and then enter with my child, as the provider is sometimes in the middle of an
activity with a child and cannot leave them unattended.
Abide by prearranged hours. If you will be running late, call us and let us know as soon as possible.
Financial agreements must be kept according to the contract. Should there be any questions regarding
your bill, we will be glad to discuss it and go over it with you. PROMPT PAYMENT IS REQUIRED as
we have a budget to follow and scheduled payments to make.
Bring a change of clothing for my child.
Provide the following for my child if necessary. Diapers, wipes, sun block, formula, baby food, change
of clothing, bottles                     
Give prompt notice of any change of address or telephone number at work or at home
